In our 13th HIT Breakdown episode, we’re going to dig into chronic care management (CCM). As a refresher, CCM is part of Medicare’s value-based-care initiatives. Here at Nordic, we’ve already helped customers implement this program within Epic using Healthy Planet. To discuss these programs, we’ve invited two senior Nordic consultants, Jake Aleckson and Jason Cassidy, who are joined by our Director of Optimization Solutions Rick Shepardson.
